Work as an independent contractor, choose the referrals that fit your availability and service area, and organize visits around your schedule.
Straightforward Compensation
Earn a flat rate of $80 per completed visit, regardless of visit type. OASIS visits, when applicable, are compensated at a separate higher rate.

Minimum Qualifications
Licensed PT, OT, or SLP
Bachelor's degree from an accredited university (Masters and Doctorate Level Preferred)
At least five years of home-health experience
Current professional license and required certifications
Professional liability
Automobile insurance
Reliable transportation
Ability to complete timely, accurate documentation
Current CPR
Current TB statement
Current Hep B vaccine proof or statement of declination

Care to Keys is currently partnering with physical therapists, occupational therapists, and speech-language pathologists. We are not accepting PTA, COTA, or STA contractors at this time.
How Therapists Can Contract With Care to Keys
1. Express Interest in Contracting
Complete our contact form and indicate that you are interested in contracting with Care to Keys.
2. Complete the Review Process
Qualified clinicians will be contacted to discuss experience, availability, service area, expectations, and current contracting needs.
3. Finish Credentialing and Onboarding
Selected clinicians must complete the independent-contractor agreement, submit all required credentials, and complete orientation before accepting referrals.
Submitting a request does not guarantee selection, referrals, patient volume, or a specific geographic assignment.
